A Hospital Passport is a useful way of documenting essential information about you and your health, with support if necessary, so that you can take it with you if you need to go to hospital.  The purpose of the Hospital Passport is to let medical staff know as much about you as they need to know to reduce the time necessary to give you the treatment you may need and to communicate with you. This is particularly important if you have communication problems.
