How it Works

An assessment, usually known as a community care assessment, is carried out on the disabled person, which should capture all of the areas where ability does not exist or is limited in some way.  Both the type of disability and the degree of it should be captured.  The outcome is called "Presenting Needs"

The next step is to compare the outcome of the assessment with the Fair Access to Care Services criteria, which will determine which items in the total need qualify for funding - these are called "Eligible Needs".

The local authority will arrange (commission) services to meet eligible needs.

There is a downloadable Community Care Assessment Checklist, which is available from this page.  This has been developed by carers and has proven to improve the quality of assessments when a copy is shared with the assessor.
